Nosebleeds can be categorized into two types, anterior and posterior. Anterior nosebleed takes place when the frontal blood vessels of nose break whereas posterior nosebleed occurs in the backward portion of nose. Posterior nosebleed involves flowing of blood through the throat and can be therefore quite dangerous.

Most nosebleeds don’t require medical attention. However, you should seek medical attention if your nosebleed lasts longer than 20 minutes, or if it occurs after an injury. This may be a sign of a posterior nosebleed, which is more serious.

Causes of nosebleed

Nosebleed can take place due to various reasons out of which, dry air is the most commonly observed. A dry environment causes dryness in tissues inside the nose that are also known as nasal membranes. Further, this dryness becomes a reason for crusting inside the nose leading to itching and irritation. Generally in due to this irritation, patient scratches his nose that causes bleeding.

Apart from the above, there exist several other causes of nosebleeds as well that majorly include;

  • Heavy doses of aspirin
  • Chemical irritants
  • Respiratory infection
  • Nasal injury of any kind
  • Picking in nose
  • Blood clotting
  • High blood pressure
  • Cancer

Nosebleeds that take place after an injury may signify skull fracture, broken nose or any other severe nasal damage.

Treatment of nosebleeds depends upon their type. Anterior nosebleed takes place in the frontal area of nose, usually through nostrils. This is not so serious and can be handled effectively by sitting up and squeezing the softer portion of nose.

Lean down forward keeping your nostrils closed and breathe through your mouth. Repeat this for 8-10 minutes and monitor your bleeding accordingly. If the bleeding remains, repeat this exercise for another 10 minutes.

Besides this, a nasal spray or decongestant can also be used. If you are unable to control bleeding on your own, visit your doctor as soon as possible as it may be a sign of posterior bleeding that requires proper medication.

Posterior nosebleed, being more serious and dangerous than anterior nosebleed involves flowing of blood through the back of nose, down the throat. It should not be treated at home.

For treating persistent and frequent nosebleeds, cauterization is used.

Apart from this Dr. Agarwal also suggests following therapies that provide the perfect treatment for sleep apnea.

  • CPAP or continuous positive air pressure is a device that prevents apnea and snoring by keeping the air pressure greater than that of surrounding. CPAP is helpful in most severe cases but is generally suggested for the moderate ones.
  • Certain oral appliances that are customized to keep throat open can also be used. These appliances bring the jaw forward keeping throat open.
  • Supplemental oxygen while sleeping also helps especially in the cases of central sleep apnea.
  • Adaptive servo-ventilation or ASV is an airflow device that records the normal breathing pattern of the patient and stores it in a built-in computer. Further, this recorded pattern is utilized to stabilize the breathing of the patient when he falls asleep.
  • Besides these therapies there exist surgical methods that cure sleep apnea permanently but according to Dr. Rahul Agarwal, surgery should be used as a last resort when all the above therapies fail to treat.
  • Tissue removal is a process in which doctor removes the tissue present between backward portion of mouth and the upper region of throat.
  • Tissue shrinkage involves removal of tissue present between the rear mouth and backward region of throat by using radiofrequency ablation.

What is Mucormycosis / Black fungus?

Mucormycosis is a fungal infection is caused by a group of molds called mucormycetes. These molds live throughout the environment. Mucormycosis is a fungal infection that is being commonly observed after the second wave of COVID-19. Fungi are present everywhere in nature and are particularly found on decaying vegetation and soil. All of us are exposed to this infection as it is caused by inhalation of spores. A healthy immune system can protect us from this Infection. Mucormycosis mainly affects people who have health problems or take medicines that lower the body’s ability to fight germs and sickness.

Technically speaking this fungus is white in colour, but the dead tissue because of its infection looks black, hence the common name “Black Fungus”

Who is at risk of mucormycosis?

Conditions like HIV/AIDS, uncontrolled diabetes, mellitus cancers, organ transplant, long-term corticosteroid and immunosuppressive therapy increase the risk of this disease. Recently Covid patients, especially those who have received steroids ( with or without oxygen supplementation or Immuno-modulatry drugs like Toclizumab) are at high risk

Is mucormycosis contagious?

Mucormycosis is not contagious, and therefore, it cannot transmit between people and animals. Individuals contract this infection by coming in contact with the fungal spores in their environment. If inhaled, the spores can infect the sinus.

What are the symptoms of Mucormycosis Infection?

Symptoms of mucormycosis depend upon the location where fungal growth takes place in the body. Based on these locations mycormycosis can be broadly categorized as following types-

  1. Rhinocereberal,
  2. Pulmonary,
  3. Cutaneous,
  4. Disseminated

Symptoms of sinus and brain mucormycosis which is commonly affected in Covid patients include:

  • One-sided facial swelling, Facial pain
  • Fever
  • Headache
  • Nasal or sinus congestion, Sinusitis
  • Purulent nasal discharge
  • Black lesions on nasal bridge or upper inside of mouth that quickly become more severe

 Is black fungus dangerous?

Mucormycosis, also known as black fungus, can turn dangerous if left untreated. You need not panic about it and if you experience any of the above-mentioned symptoms, visit your doctor as soon as possible.

Precaution + A strong immune system = No mucormycosis.

What is Treatment of this disease?

A high degree of suspicion is required in early stages due to its non-specefic symptoms and if treated early then results are very rewarding. Hence an early consultation with an ENT surgeon is required. Your ENT surgeon may advise an MRI / CT scan of PNS and brain. He will do a Diagnostic nasal endoscopy and may take a swab (for KOH) or Biopsy if needed. Initial treatment is medical, but with advance stage and dead tissue, surgical debridement (Removal of fungus) is needed.
